Prison Fellowship/Angel Tree Scholarship Application

PLEASE READ | This form is only to be filled out if you are applying for an Angel Tree Scholarship.  Angel Tree® Camping makes camp possible for children of incarcerated parents by providing scholarships and partnering with churches and camps nationwide. Angel Tree/Prison Fellowship scholarships are limited to one session per summer per camper. Angel Tree/Prison Fellowship scholarships do not cover Camp Store credit.   Attendance to camp must be confirmed with the camp office before May 15. Registration will be canceled if not confirmed. A deposit of $25 is required. If the camper attends camp, the $25 deposit will be converted into store credit for them to use during their stay. To submit your deposit, please mail a check to Cold Springs Retreat 478 Acklin Gap RD, Conway AR 72032, or call 501-513-7622 to pay by phone.  Please respond to the items below as best as you can.
