This Access Request Form must be completed to gain access to the STTARR's Imaging Facility in TMDT 7th Floor. Once reviewed and approved by the STTARR Steering Committee, this application will be routed to the UHN Animal Resources Centre (ARC). This application must be kept current with corresponding Animal Use Protocols (AUPs) and/or Biosafety Certificates (BCs) as well as Radiation licenses & permits. Imaging sessions may be scheduled ONLY after this application has been approved by STTARR and ARC. Please Note: For studies involving live animals or biohazardous material, the applicable AUP/BC (s), including amendments, must be approved prior to the scheduling of imaging sessions online. If animals additional to those already approved on the applicable protocol are needed for this imaging session, the corresponding AUP protocol will need to be amended prior to the on-line scheduling of imaging sessions.
