Luau Dance Permission Slip
A parent or guardian is required to complete this form for each attendee to the TVHS Luau Dance before 8PM on Friday, August 18. Read each statement carefully. 
The Luau Dance will take place on the TVHS Campus on Friday, August 18 from 8PM to 10:30PM. All attendees must check-in prior to 9PM and are required to be at the event until at least 9:30PM. There will be dancing, photo booths, food, and other forms of entertainment such as video games and carnival games.
Attendees are required to abide by the same rules and regulations in place during the school day (i.e. attire, behavior, possession or use of prohibited items).
