Once you apply to a post-secondary institution, please complete this form so that we can provide your high school transcript to the school, employer, organization, etc. In order for a transcript to be deemed official, the transcript must be sent directly from us to them, or they can be provided to the college by you in a signed and sealed envelope. You may also request an Unofficial Transcript for your records and we will stamp them as "Unofficial". Please indicate your preference on this form.
