AFTER SCHOOL PROGRAM

* Program begins September 9th and runs through June, Monday thru Thursday following the Montpelier-Roxbury school calendar, including closures for weather.

* The monthly rate is due on the first of each month. Checks can be made payable to: Vermont Artpath and either mailed to: Ellis Jacobson, 288 Cityside Drive, #63, Montpelier, Vermont 05602 or delivered in person on the student's first day of the month.

* The monthly rate is constant without compensation for weather closures, vacations, or absences.

*The student arrival time is between 3:00 pm & 3:15 pm. Pick-up time is no later than 5:30 pm.

* Late Pick-up fee for repeated incidents is $10.00 with an additional $5.00 for every 15 minutes after 5:45 pm.

* The program Director, Ellis Jacobson, must be notified in advance of any absences or late arrivals by emailing: vermontartpath@gmail.com or calling 802 229-6178 (before 2:00 pm). Last minute absences should call 802 279-6796.

* Students should bring a daily snack and appropriate outdoor clothing for the weather.

* A student may be released from the program if behavior is inconsistent with the teacher expectations on a repeated basis and following sufficient warnings.

* No child will be allowed to be picked-up by an adult other than those listed on the registration form unless previously communicated to the program director.

* The program director must be notified if there are any changes to the student's information on this form.

JULY VACATION CAMP

* Puppet & Puppet Show Creation runs JULY 7th thru 11th, 9:00AM - 4:00PM

* There is a non-refundable deposit of $150 to be mailed at the time of submitting the online application to: Ellis Jacobson, 288 Cityside Drive, #63, Montpelier, Vermont 05602. Checks can be made payable to: Vermont Artpath or to Ellis Jacobson.

* Daily arrival time is at 8:45 to 9:00 AM. Pick-up time is between 4:00 & 4:15.

* Late Pick-up fee is $10.00 at the discretion of the instructor with an additional $5.00 for every 15 minutes after 4:30PM.
