ATTENDANCE REQUEST FORM OCTOBER 13th
Hello Flying Samaritan Member!

You have been selected to volunteer for next week's orphanage trip (October 13th). Please reply to this email by Monday, October 8th at 11:59PM to confirm your selection.

We will be having pre-clinic meeting on Thursday 10/11 from 6:00pm - 7:00pm at Geisel Library, Room 1045 of 1st Floor West. Attendance is MANDATORY for all members and officers, even if you have previously attended a clinic. If you have a schedule conflict, please contact me and I will coordinate with you to reschedule. At the meeting, please bring your clinic fee ($30) in exact change. If you fail to RSVP, pay in exact change, or attend the meeting (and do not attempt to reschedule), you will be dropped from the trip. Our departure time on Saturday will be announced at a later date.

As a reminder, the requirements for active membership are attendance of 2 GBM's (or bringing a friend to 1), and attending 4 other Flying Sams events. Failing to do so will disqualify you from future application cycles.

Can you drive volunteers to and from clinic? *
Around 4.5 hours of driving total. Drivers do not pay clinic fee. Drivers receive $30 gas reimbursement, $10-20 for toll, and $15-$25 Mexican Insurance reimbursement (with proof of purchase). Please utilize safe driving practices.

After entering your info and once your are directed to the Communication page, to pay the $10 fee, please go the Palomar Section, Select "Yes" for Membership, and press the gray button "Pay-10".
