Information if you fill out a CEU & evaluation:  
By filling out this form you are receiving a Continuing Education Unit (CEU) through the Northwest Indian College (NWIC).  CEUs are non-credit units and are typically utilized for community events, such as conferences, workshops, online learning, and where appropriate large events. DIRECTIONS:  
1.) Be 16 years or older to participate.
2.) If you are NEW to filling out a CEU, verification of identity will be requested by NWIC.  
3.) Fill out each section the best you can.  Contact us at CEU@nwic.edu if you have difficulties with the google form.
4.) Make sure you provide an email or phone number for the CEU certificate of completion.
