You should have your Room or Apartment Condition Inventory (found in mySLU under Tools>Student Housing) and student account bill available. If you experience any trouble with this form, please send your information, including your name, Banner ID, description of appeal, and rationale for appeal to firstname.lastname@example.org. You may also send copies of any supporting documentation (for example, emails or photos) by email to email@example.com or via the postal service (221 N. Grand Blvd., DuBourg Hall, Room 157, St. Louis, MO 63103), as you are not able to upload those documents into this form.
Please remember you will not meet in person with the Damage Appeals Committee, and the information you submit as part of your appeal will be the only information the Damage Appeals Committee will consider.
IMPORTANT: You should not wait until the appeal process has ended before paying your University account balance. You should pay your account balance when due to avoid finance charges. If your damages are reversed, you will not receive a credit for any finance charges incurred as a result of an unpaid balance.
