Guidelines for receiving your free Portfolio Report Card*:

• Step 1: Submit your response to the questions on this form
• Step 2: Wait for notification your Portfolio Report Card results are ready
• Step 3: Book final exam appointment with the financial advisor that graded your investments
• Step 4: Pick up your Report Card on your scheduled final exam day at the advisor’s office near you and claim your $100 if you score “A”

The Portfolio Report Card is always personally delivered by a licensed financial advisor who will explain your results. The local advisor assigned to analyze and grade your investments will also discuss how to fix any flaws.
