ONLINE ENROLMENT FORM
Procedure:
1. Fill out the enrolment form.
2. You will receive an email for your assessment.
3. If payment has been made, please scan or take a picture of the deposit slip alongside with the student's name, grade, and branch. E-mail it to hca.enrolment@gmail.com 
4. OR will be issued upon verification and can be picked up upon resumption of office work.
5. For NEW STUDENTS, please submit the following requirements:                                                                                
- Photocopy of PSA Birth Certificate
- Certificate of Good Moral Character (Grade 4 to Grade 12)
- Form 138 (SF 9) / School Report Card of the previous level
- Form 137 (SF 10) / Learner's Official Transcript of Record
