This form will collect the information of all nominees intending to serve as a Board Member on Quidditch Canada's Board of Directors. All of the information collected in this form will be shared with Quidditch Canada's Associates - registered team/club organizations with Quidditch Canada, in order to help them make an informed voting decision.

This nomination closes on February 18, 2018. Quidditch Canada's first elected board will be declared at the Annual General Meeting on Sunday, Feburary 25, 2018.
