Elementary Delivery Model Change Request for Spring Semester 2021
The Peach County School System is collecting requests from parents who wish for their students to transition between in-person and online learning going into the 4th grading period, starting on March 11, 2021.

Online and In-person learners who wish to change delivery models should use this form to submit those requests. No action is required for students who intend to continue with their current delivery model.

Please complete one form for each online learner in your household and/or child requiring a change in delivery model no later than February 26, 2021.

No changes will be made until March 11, 2021.

The time period between now and February 26th is the last opportunity parents will have to select a delivery model for their child(ren) for the remainder of the 2020-2021 school year. This option is only available for elementary students. Middle School and High School students are required to stay in their selected delivery models.

In order to provide adequate time for schedule changes to be made, all requests must be submitted by February 26. Notification of approval will be provided by March 5th.
