Please fill this out for the 26-27 School Year! You must fill out this form for each pet.

This application does not apply for animals considered to be an ESA. If you are looking for information on the ESA Policy & Procedure, please click here.

Before completing this application, all pet owners must read and be familiar with the ACU Residence Life Pet Policy. Please read thoroughly to ensure you understand all requirements.

Pet Application Deadlines by Term are as follows:

Fall Term: All Pet Application materials must be received no later than August 1st.
Spring Term: All Pet Application materials must be received no later than December 1st.
Summer Term: All Pet Application materials must be received no later than April 1st.

Please ensure any applicable roommates have completed the Pet Roommate Agreement prior to your submission of this application. Any applications submitted without completed Pet Roommate Agreement(s) will be denied.

Pets other than ESAs are only allowed on the first floor of Smith-Adams Hall!

All veterinary documentation must be submitted at the time of your application. Any missing or insufficient documentation will automatically result in the denial of your pet application. The following must be included in your submission:

Current vaccination records
Spay/neuter certificate or documentation
Certificate of health/wellness
Records that verify pet’s species, breed, gender, weight, and age

Pet applications are reviewed on a weekly basis. Residence Life Staff will review your application and corresponding materials and will notify you regarding the status of your application within ten (10) business days from the date of submission.
