Please fill out the attached form if your child is unable to attend tryouts. You must submit this form separately for every child. For example, if you have 2 children missing tryouts, you must submit this form twice.

PLEASE NOTE: Even though your child or children are unable to attend tryouts you MUST register them for tryouts because if they are offered a spot, this is how I assign them to a team.
