Group Visit Request Form
Please complete the following request for school, organization, and community group visits to Discovery Center Museum in Rockford, Illinois. Group rates and programs are only available to groups that complete a Group Visit Request form at least 14 days prior to their earliest requested visit date.

When you are finished filling out the form, click "submit" to send your request to the Museum Office. You will receive a copy of the reservation request at the email you provided in the request form. Submitting a request does not guarantee a Group Visit on one of the dates selected. Additional correspondence will be sent to the email provided about confirming your group visit.

Please note that a reservation is required for group visits. Lunchroom use, group planetarium shows, Science Demonstrations and STEM Labs are not available without a reservation.

Lunch Plan

Discovery Center Museum does not have on-site food service. Groups planning to have lunch at the Museum will need to reserve an eating space and bring sack lunches; refrigeration is not available. Our lunch room spaces can accommodate up to 60 people.  If your group is larger than 60, your group will be divided at lunch time.

STEM Lab/Planetarium/Live Science Demonstration Add-ons

Please select the STEM Lab, Planetarium Show, or Science Demonstration that you are interested in adding to your group visit. STEM Labs run 45 minutes and are geared for students in K-8th grades. Each STEM Lab session can accommodate 25 students; each session is $75. Each Group Planetarium Show can accommodate 25 students. Group Planetarium Shows are 30 minutes long; each show is $40. STEM labs are available during the school year only. Science Demonstrations are 20 minutes long and can accommodate 90 people; each show is $100.

Please allow two business days for your group reservation request to be processed. To cancel or alter your reservation please email fieldtrip@discoverycentermuseum.org. Any changes to your reservation need to be made 14 days prior to your scheduled visit date. Groups agree to abide by Discovery Center Museum's Group Visit policies.
