Off-Campus Student Lunch Permission and Waiver of Liability Form
Elementary students in Northbrook School District 28 can go home for lunch. This form is valid only for purposes of a student leaving the School District's premises in order to eat lunch at his/her residence at the request of a parent or guardian. This form must be approved by the student's Principal and a separate form must be completed for each student. 

Northbrook School District 28 requires completion of the following waiver in order for your child to leave the school district building during his/her regularly scheduled lunch period to travel to his/her residence, without school supervision, to eat lunch.
