I, the undersigned, being parent, legal guardian, or parent in charge of the above named minor participant, in consideration of being permitted to participate in the sport league conducted by the Boys and Girls Club of the Peninsula, do hereby give permission for said minor to participate and do release the Boys and Girls Club of the Peninsula of any and all liability or responsibility arising from any and all injuries received and incurred by participating in this activity./ Yo, el firmante, soy padre o tutor legal a cargo del participante menor mencionado anteriormente, en consideración de que se le permita participar en la liga deportiva dirigida por el Boys and Girls Club de la Península, doy permiso para este menor de edad para participar y liberar al Boys and Girls Club of the Península de cualquier y toda responsabilidad o responsabilidad derivada de cualquier y todas las lesiones recibidas y en las que se haya incurrido al participar en esta actividad.
