Please use this form to report credits earned between January 1 - December 31, 2024. Credits may have been earned online or in-person. Please use the COMMENTS boxes to add any additional information not captured by the pre-filled lists.

This form does not calculate the credits for you - so you may want to jot down the # of credits earned in each section so you can add them easily at the end!

10 CREDITS ARE REQUIRED ANNUALLY. This must include 2 Education and 2 Safety credits.

IMPORTANT: You must always have at least two safety credits. If you have "extra", safety credits, these can be counted toward your education credit requirements. However, education credits cannot be used to fulfill your safety requirement.

DEADLINE: February 15. After February 15, a $50 late fee will be added.

MAA MEMBERS: Recertification is included with your MAA membership. Please make sure that your dues are paid for 2025. Your dues invoice is located in your account at MassArbor.org.

Non-MAA MEMBERS: Non-members pay $95 for recertification. This can be paid online in your account at MassArbor.org.
