Meal Request Form

To help us buy and prepare the right amount of food, please fill out this order form.

We offer free meals for any child 18 and younger through Thursday, Aug. 20.

Meals will be available for pickup Tuesdays and Thursdays. On Tuesdays, each child will receive 1 hot lunch, 1 cold lunch and 2 breakfasts. On Thursdays, each child will receive 1 hot lunch, 2 cold lunches and 3 breakfasts.

Please note:

- Meals are available to children 18 and younger.
- Children do not have to be students of CK Schools.
- One meal package per child.
- Children do not need to be present for meal pickup.
- Adult meals are not available for purchase.
