PLAYER WAIVER & RELEASE

By registering with AZ United Volleyball Club, I acknowledge and understand that participation in volleyball activities, including open gyms, tryouts, practices, tournaments, games, conditioning, and other club-related events, involves inherent risks of injury. I voluntarily assume all risks associated with participation and agree to release and hold harmless AZ United Volleyball Club, its coaches, staff, volunteers, directors, and representatives from any claims, liabilities, or expenses arising from injury, illness, or loss that may occur as a result of participation, except where prohibited by law. I confirm that the player is physically able to participate and that I will notify the club of any medical conditions, allergies, or other concerns that may affect their participation. In the event of an emergency, I authorize AZ United Volleyball Club staff to seek appropriate medical attention for the player when a parent/guardian cannot be reached. *
