Complete this trip record for every wheelchair trip not paid for with a Paratransit Taxi Debit Card assigned to a wheelchair rider.  Only record trips that require the securement of a wheelchair.  Wheelchair trips will be verified with video review.  Form should be filled out at end of trip and must be filled out within 2 hours of trip completion to be accepted.
