Please read the following information carefully. We are happy to answer any questions or concerns you may have. IMPORTANT NOTE: Filling out and signing this contract indicates and acknowledges the adopter has read and understands any and all conditions set forth by Halfway There and waives the right to protest this contract due to misinterpretation or misunderstanding.
NOTIFICATION: I understand and agree to notify Halfway There immediately if anything happens to the animal that would require his/her destruction, relocation, etc.  VACCINATIONS AND VET CARE: I understand and agree the animal must be kept current on all vaccinations for the balance of his/her life. I commit to at least annual vet visits, and other expenses as deemed necessary to sustain a positive quality of life.
IF NOT YET SPAYED/NEUTERED PROOF OF SURGERY MUST BE PROVIDED BY:
MONTHLY PEST MEDICATIONS: I understand and agree to provide year-round pest control medications, making certain these medications are in accordance with the animal’s weight. Dogs are to be kept on year-round heartworm prevention medication. While year-round maintenance is preferred, flea and tick control medication is required through at least the summer months of May-September. LIVING CONDITIONS: All Halfway There animals are inside animals. Therefore, I understand and agree to keep the animal primarily indoors. Dogs are NEVER to be tied or chained outside, nor left outside for extended periods. When dogs are outside, they must be kept in a fenced area or on a leash.
MICROCHIP AND IDENTIFICATION: The animal has been implanted with Microchip # _______________________________through the _________________________________company.
I understand and agree to keep the company informed if my contact information changes. Halfway There remains the secondary contact on the Microchip for the life of the animal. RETURNS: I understand and agree that anything that would prevent me from abiding by any of the terms of the contractual agreement would require the return of the animal to Halfway There. Additionally, adopter will notify Halfway There if the Adopter no longer wishes to own said animal for ANY reason. Returns are accepted unconditionally and the adoption fee is non-refundable. Note: I will under no circumstances post the pet for adoption on the Internet or surrender the pet to any other organization, such as an animal shelter, where the animal is likely to be destroyed. I will not surrender the pet to another individual without the express written consent of a Halfway There board member.
DISCLAIMERS: I agree and understand that the Halfway There shall have the right to and may repossess the pet in the event that the pet is not spayed/neutered by the date stated above or in the event that any of the above stated conditions are not met. The respective parties agree and authorize any constable, humane officer or agent of Halfway There to enter the premises where the pet is located, take possession of the pet and deliver the pet to Halfway There without the necessity of any court order or further proceedings of any kind. I agree and understand that Halfway There makes no warranties, representations or guarantees whatsoever as to the adult size, temperament, characteristic or health of the pet and hereby disclaims all implied or expressed warranties of fitness or health. I also understand that Halfway There will not be responsible for any medical care or costs thereof required after the placement of the pet. I agree that Halfway There will not be held responsible for any sickness incurred by the pet before or after receipt of the pet.
IN CASE OF EMERGENCY SITUATIONS, such as a ‘lost’ animal reported to us, please indicate a name and number of a contact person who can take a message for you in the event Halfway There is unable to reach you.

RESPONSIBILITY: I understand and agree that as the adopter, I will assume responsibility and liability arising from ownership of the animal. Again, I understand and agree to ALL terms of the adoption contract, and realize that failure to comply can result in the repossession of the animal.
