This is the name of the organization or business that will be managing this project. If there is no managing organization and you are applying as an individual, please write "sole proprietor".

If you are applying through your business, put your business address. Otherwise, put your home address as your response.

Corporate Status

What is the current corporate status of the legal entity? If you do not have a legal entity, put "sole proprietor" as your response.

Project Name *

Please name the project that is seeking fiscal sponsorship. If the project is managed by a business, then the name should differentiate the project from the business.

Project Summary *

Provide a short description of the mission of the project, who you intended to serve, and your plan of action for accomplishing your goals.

Fundraising Plan *

Provide a short description of how you intended to raise money to support the work of the project.

Business Description *

If the legal entity managing this project intends to start or continue operating a for-profit business in addition to managing the project, please include a short summary of your current or proposed business activities. (If you do not intend to operate a for-profit business, you can answer "n/a")
