NCS School Community CARE Form
Students who are issued a behavior consequence should complete the NCS School Community CARE Form. 

Students, before you submit your completed CARE form, the teacher issuing the form must approve your responses. If the teacher does not approve the form due to lack of student ownership and responsibility, then students will be required to complete the form again. Refusal to complete a CARE Form will result in an increased consequence.
