Waves 4 Readiness Checklist
NB. This form MUST BE completed for all the remaining Parishes wishing to be considered for Wave 4 proposals of designation. Otherwise, all remaining lane nominations will default to the backlog register for potential future programme or available funding.

Please indicate your parish readiness for QL designation by answering these questions below:
> does the online master map show the correct lanes with start & end points
>  have you already completed and sent a signage spreadsheet, showing all sign locations & co-ordinates
> date when your parish council has or will hold your open public consultation
> date of parish council meeting confirming the lanes to be put forward for designation
> above relevant files to be loaded on to the project google drive
> do you require attendance by a QLS team member or another volunteer to a parish or open meeting?
> any further help, such as signage locations, spreadsheet completion, parish consultation
> date of minutes confirming your parish voluntary contribution upon successful designation
