Thank you for taking the time to complete the suspension request form.

Please take a quick read before proceeding to the suspension request form. Your suspension should be considered completed once you receive your "suspension receipt".

Freezing your membership:
Per the terms of your membership, we provide the opportunity to "freeze" or "suspend" your membership a maximum of 3 times per calendar year.
You may freeze your membership for a minimum of 10 days per suspension and a maximum of 30 days per suspension.
All suspensions require a suspension form and a confirmation suspension receipt to be sent to you.
You may not cancel your membership while your account is suspended. Any cancellation while your account is suspended will be considered as 30 days notice from your return date, per the terms of your membership.

I understand and agree, I am responsible for monitoring my suspension dates. My account will un-suspend on the requested Freeze End date, and my payments will resume. If my dates change, I am responsible to notify the Studio, via email, of my new return date or my account will un-suspend automatically, and my monthly payments will resume. *

Suspension Extension Requests: If you DID NOT choose a max 30 day long suspension, and need to extend your dates, we would be happy to adjust your dates as long as they are within the 10 day minimum and 30 days maximum date range. Simply email us and we'll send you a Suspension Extension Receipt. All other requests are considered exceptions and require a manager's approval. *
