Fees can be brought to the first day of camp. Please make checks payable to Vale Basketball.

Cost: $50 for single participant ($45 for extra siblings). Bring first day of camp.

By signing this enrollment form I am consenting for my child to participate in the Vale Vikings Basketball Camp. I/We know that participation in this activity may result in some injuries, and I do hereby waive, release, absolve, indemnify and agree to hold harmless Vale School District and coaches. For any claim arising out of any injury to my child sustained during this camp or transportation thereto: or hold the above named persons liable for seeking or obtaining aid and assistance to hospitalization medical aid, or ambulance services for participants in this program who are believed to injured or hurt.
