Purpose and Goal

NKY Health is committed to providing a workplace where all employees can feel free from threats of physical harm.

To that end, all employees and supplemental staff receive annual training about actions to take in order to prevent and prepare for potential active shooter incidents, when confronted with an active shooter, and when responding to law enforcement officials.

The goal of this form is to confirm that the training was completed and to affirm understanding of the content and related policies.
