TGI Athlete Screening
The safety of our athletes is our top priority. In order to prevent the spread of the coronavirus and reduce the potential risk of exposure all athletes are required to complete this questionnaire before every class or training and entering Toronto Gymnastics International. Please submit this survey no more than 24 hours in advance of the class.
• Anyone who has had symptoms of COVID-19 in the last 14 days must self-isolate at home.
• Anyone under the direction of the public health officer to self-isolate must follow those instructions.
• Anyone who has arrived from outside of Canada must self-isolate for 14 days and monitor for symptoms.
Please follow the instructions listed with each question if you answer "YES" to any of the questions. Please do not come to the gym for training today.
Please make sure that the confirmation email is sent to the person dropping off the athlete.
