This form is for renewals only.
If you’ve had a Max Card from us before and need to renew, you’re in the right place!

Eligibility
Your child is eligible for a Max Card if:

You live in the Bradford Council district, or your child attends a school in Bradford.
(If you live elsewhere, just tell us the name of the Bradford-based school your child goes to.)

Your child receives DLA or PIP, at any level.

How to renew your card
If you meet the criteria above, complete the form below to renew your Max Card.
💷 Each card costs £5.50 (including postage).

What evidence do we need?
To complete your renewal, please attach a clear photo or scan of your child’s DLA or PIP letter.
✅ The name must be clearly visible.
We’re unable to process applications without valid evidence. Once we've verified your documents, they’ll be securely deleted or destroyed.
👩👧👦 Got more than one child with additional needs?
You can apply for more than one card — but we’ll need DLA/PIP evidence for each child.
📅 Max Cards can be issued up to age 23, and are valid until age 25.
⏳ Please allow up to two weeks for your card to arrive after your payment is received.
🤝 Automatic Membership
By filling in this form, you’ll automatically become a member of the Parents’ Forum for Bradford and Airedale.
That means you’ll receive occasional updates about our meetings, activities, and helpful information related to disability and SEND.
We’ll also use details like your postcode, ethnicity, and your child’s age, needs and education type to help monitor and improve our reach and representation.
