It is my choice to receive yoga therapy and I recognize that it promotes the well-being of my body and mind. I acknowledge that yoga therapy does not replace professional medical care, and it is not a substitute for a doctor's examination or diagnosis. It is advised that I consult my primary health care provider for medical services. Information shared during a yoga therapy session is educational in nature intended to support awareness of my health, and I am encouraged to use it at my own discretion.

I recognize that I must be in adequate physical and mental health to participate. I understand that yoga includes physical movements as well as an opportunity for gained awareness, and relief of muscular tension. I recognize that this may require some physical exertion, which may cause physical injury, and I am fully aware of the associated risks and hazards.

If there is any doubt in my ability to participate, it is my responsibility to consult with a physician beforehand and obtain permission. I understand that Maya reserves the right to refuse service in her sole discretion if she deems me to be physically or mentally unfit to participate for safety concerns.

Furthermore, I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result of participating in these sessions. I agree to inform Maya of any physical limitations, physical discomfort and/or injuries before or during sessions, and I take full responsibility for any nondisclosure.

I knowingly, voluntarily and expressly waive any claim I may have against Maya Lake-Freeman for any injury or damages that I may sustain as a result of participating in sessions as stated above. I have read the above release and waiver of liability and fully understand its content. I voluntarily agree to the terms and conditions stated above.
