23-24 Name Change Request for Yearbook Portrait

This form is for families to request a CHANGE for their student's first name. This request form is NOT for students' nicknames (e.g. Daniel to Danny). This request form is for students whose name is not the name they go by in Skyward (e.g. from Alexander to Taylor). If you have any questions, please email minnetonkayearbook@gmail.com.

Again, this form is NOT for nicknames. This form is also not for last names; please reach out to the main office if you need a last name changed in Skyward.

This form is to be filled out by parents and guardians ONLY (must be parent email - not MTKA).

The final deadline for this change is November 1, 2023. After that, we will be publishing and printing the portrait pages and will be unable to make changes to names. Please see minnetonkaschools.org/yearbook for a full list of FAQs and procedures.
