Please use this form to nominate an outstanding educator for the SBPS Foundation's Excellence in Education Award.  Any SBPS employee is eligible to receive recognition.  Recipients will be announced three times-a-year, in October, January, and April.  Recipients are not eligible to receive award in consecutive years.  
 
Nominations will accepted throughout the year and anyone may submit a nomination.  Nominations will be kept confidential, but please provide the following information so we may contact you if more information is required.  Thank you!
