SKCFC South King County EFAP Pre-Screening Questionnaire

Emergency Food Assistance Program (EFAP): A state-funded program that supports local emergency food providers with funding to help alleviate hunger. The funding is flexible and may be used for the purchase of food, operational costs, nutrition training and equipment or repairs. 

South King County Food Coalition serves as one of three lead agencies for the King County allocation of EFAP funds.

Add definition of "unduplicated" means each household/individual is only being counted one time and "duplicated" means how many repeat visits were made. For example, if a family of four came to the food bank 4x in a year they would have made one "unduplicated" visit and three "duplicated" visits. Add these two numbers together to receive the total households/individuals. This would be noted as four "unduplicated" individuals and twelve "duplicated" individuals.

EFAP funding in South King County will be reimbursement based only. Is your organization able to operate on a reimbursement basis? *
"Reimbursement" means your organization will directly pay bills and then request repayment from SKCFC. Reimbursement can take up to two months to arrive.

Funding Match information
Your program must equally match the EFAP funding allocated to your organization with funds from other sources.

There are two types of match:
-Cash Match: this is funding your organization has received from another source, which could be another government contract, a grant award, or donations/fundraising. 
-In-Kind Match: this may include the value of foods from TEFAP, CSFP, and other programs serving EFAP clients and food pantries, the value of services or labor provided on a volunteer basis, and/or the value of transportation donated. WSDA's current in-kind rates are $1.82 per lb of food, $16.69 per hour of volunteer services, and $0.56 per mile of donated volunteer mileage. 

Your organization can meet the match requirement in two ways:
-100% match. At least 50% must be in cash; the remainder can be in-kind. For example, if your EFAP allocation for the first year is $10,000.00, then your organization must be able to provide documentation of at least $5,000.00 funding from other sources plus $5,000 worth of in-kind support.
-200% in-kind match. You can meet the match requirement with in-kind support, but it must be 200% of your funding amount. For example, if your EFAP allocation is $10,000.00 then you would need to provide documentation of $10,000.00 in in-kind value from donated food, volunteer hours, volunteer mileage, or other in-kind support.
