This is a form to fill out when you have made two separate orders and wish to combine them. Filling this out will allow me to offer you a refund for shipping on the second order that you wish to combine with the first. I will try to get you the refund as quickly as possible.

> If both your orders combine exceed $100 USD you will be qualified for free shipping and I will refund you for both shipping fees. 

> If it's less than $100 USD then I will refund you for your 2nd, 3rd, etch order's shipping fees.
