ACE Mentor Program of Oregon Student Application 2021 - Part 1
The ACE MENTOR PROGRAM is a free after school program for high school juniors and seniors.

Students who are accepted into the program will be assigned to a TUESDAY, WEDNESDAY or THURSDAY team.

When: The program runs from Jan. 12, 2021, to May 4, 2021. Weekly team meetings will be held virtually. Tuesday and Thursday teams will meet from 4:30–5:30 pm; Wednesday teams will meet 6:00-7:00 pm.

Application deadline: November 20, 2020 (parts 1 & 2 completed).

Notification: Students will be notified about acceptance to the program by December 10, 2020.

There are 2 parts to the ACE Mentor Program of Oregon application:

Part 1 (Oregon): Complete and submit this application.

Part 2 (National): Complete the national online registration at https://app.acementor.org/login. This part includes the required parent/guardian consent form.

Student Requirements
•Junior or senior in high school (grades 11 & 12)
•Minimum 2.0 GPA
•Have successfully completed 2 years of high school level math
•Have a good school attendance record
•Be drug/alcohol free

ACE Attendance Policy - Regular attendance is essential to the success of each ACE team. ACE has the following attendance policy:•Students are expected to be on time and attend all meetings.• In the event of illness or unavoidable conflict, two “excused” absences are allowed. Students must notify ACE staff or their Mentor Captain in advance.• Additional absences may result in dismissal from the program.• A “no-show” is an unexcused absence and may result in dismissal from the program. *
