Do you understand the following statement: 

It is my understanding that upon successful completion of the initial training, I may be involved in providing direct services in my chosen or appointed volunteer area.  The training consists of an orientation session and “on-the-job” supervised volunteer experience.  Additional training sessions may be scheduled throughout the year.

In order to provide a clear understanding of the expectations of the Hospitality House, each volunteer is asked to read and sign the following agreement.

Do you agree to the Harassment Policy? 

A respectful environment is essential to the well-being of volunteers. Harassment will not be tolerated. Harassment includes: verbal, physical, visual, and sexual harassment. Actions or conduct based on an individual’s race, color, religion, sex, gender, gender expression, sexual orientation, national origin, age, or disability is a form of discrimination that violates the law and Hospitality House policy.

If a volunteer believes that he or she being or has been unlawfully harassed, the volunteer should immediately report the perceived harassment to your supervisor. If your supervisor or other staff member is involved, the Executive Director should be notified.

All reports of perceived unlawful harassment will be investigated. If harassment has occurred, appropriate disciplinary action up to and including dismissal may be taken. Under no circumstances will the investigation be conducted by or under the direction of the person reported to have engaged in the alleged harassment.

Retaliation against volunteers who report perceived unlawful harassment, or who participate in investigations as witness or in other capacities, also violates the law and Hospitality House policy. Such retaliation is prohibited, and should be reported immediately, following the notification hierarchy outlined above.

Do you agree to our confidentiality policy? 

Federal law and regulations protect the confidentiality of interacting with clients and client records maintained by Hospitality House Generally, the agency may not say to a person outside the agency that a client attends the agency program unless:
   The client consents in writing.
   The disclosure is allowed by a court order.
   The disclosure is made to medical personnel in a medical emergency or to qualified personnel for research, audit, or program evaluation.

Violation of the federal law and regulations by a program is a crime.  Suspected violations may be reported to appropriate authorities in accordance with federal regulations.

Federal law and regulations do not protect any information about a crime committed by a client either at the agency or against any person who works for the agency or about any threat to commit such a crime.

Federal laws and regulations do not protect any information about suspected child abuse or neglect from being reported under state law to appropriate state or local authorities.

•    The most important thing to remember when talking with folks on the phone is client confidentiality. We do not give out the resident status of any of our clients, unless the client has given us specific permission to do so. When people call asking for someone other than staff, the best thing to say is
“I’m sorry, but due to our confidentiality policy, I am not able to even confirm whether or not that individual is staying with us.  I can take a message and post it on our bulletin board and if that person comes through or is staying here, they will see it.”
This goes for people who are not staying here as well.  Let them know that you will take a message, post it, and if that person is here, they will receive it. 
•    None of the resident’s telephone numbers are to be given out. This includes the lobby telephone. The main number 828-264-1237 may be given out

Note to students:  In the past, some professors and teachers have requested use of information gathered in reports or summaries.  Reports should not contain accurate names of residents, clients, guests, etc. (use of staff names is fine) without their own permission, preferably in writing.  Initials and pseudonyms of guests are generally acceptable but may not be allowable to your specific class.  Please ask your professor or department chair to contact the Hospitality House if they have any questions or concerns in this area.

I acknowledge that I am voluntarily participating in activities at Hospitality House of Northwest North Carolina (“Organization”). I understand that volunteer activities may include, but are not limited to, indoor and outdoor work, lifting and moving items, cleaning, organizing, food service, grounds maintenance, and other related tasks.

I understand that participation in volunteer activities may involve inherent risks, including but not limited to slips, trips, falls, exposure to weather conditions, contact with debris or discarded materials, and other unforeseen hazards.

I voluntarily assume all risks associated with my participation.

In consideration of being permitted to volunteer, I hereby release, waive, discharge, and hold harmless Hospitality House of Northwest North Carolina, its directors, officers, employees, and agents from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness, damage, or loss that may occur as a result of my participation, except in cases of gross negligence or willful misconduct.

I agree to follow all safety instructions provided by staff and to refrain from engaging in any activity that I believe to be unsafe.

I certify that I am physically able to participate in volunteer activities and understand that I am responsible for my own medical insurance coverage.

I have read this Release and Waiver of Liability, fully understand its terms, and sign it voluntarily
