Please complete the form below to schedule the Concussion Prevention Kit. This interactive game helps children between the ages of 4-13 years old understand the importance of wearing a helmet when participating in all wheeled actvities and certain sports.  Please be aware that you must pick-up the kit on either the day before or the day of your event. Additionally, return the kit at least three days after the event. The pick-up location is the Bradshaw Institute located at 255 Enterprise Blvd, Greenville, SC, Suite 110. You will receive a confirmation email in at least one week of your form submission indicating your date approval. Kits must be returned at least three business days after the event.
