PAYMENTS ARE MADE SEPARATE FROM THIS REGISTRATION BY CHECK OR USING PAYPAL. CHECK: Please make checks payable to BIAOH, note "2019 Summit Registration" and the name of the attendee(s) on the check or in an accompanying note, and send to BIAOH, P.O. Box 21325, Columbus, OH 43221. PAYPAL: Please access PayPal using our website, www.biaoh.org, and clicking on the "donate" button at the top of the home page. Please note "2019" Summit Registration" and the attendee name(s) in the "special instructions to the seller" field. Payments must be received via PayPal or sent by mail postmarked no later than February 15, 2019 to take advantage of discounted rates. Registration is first come, first served and confirmed upon receipt of payment and registration.

Photo Consent: BIAOH may take photos and/or make audio or video recordings of this event for use on the BIAOH website and other BIAOH materials and publications. Registration and attendance at this event constitutes an agreement by the attendee for BIAOH to use attendee's image captured during this event.

Cancellation and Refund Policy: Attendees needing to cancel must do so in writing via email sent to esmith@biaoh.org no less than 7 business days prior to the event and will be subject to a $30 service charge. Substitutions may be made up to 7 business days prior to the event and are not subject to a service charge. We regret that refunds cannot be given for cancellations received fewer than 7 days prior to the event.
