2023 - 2024 Faith Formation Program Registration Form
Welcome to a new year of Faith Formation!

Please fill out the form below to register your child for this year's Faith Formation programming. A separate registration form must be completed for each child.

If you have a child in Grades 2 or 9, please fill out the Sacramental Preparation Registration Form corresponding with your child's grade. Do not fill out this form.

Before registering your child, please review this year's welcome letter, program information, and calendar.

All families registering for Faith Formation must first be registered members of Holy Name Parish. If you have any questions about your parish registration or need to register, please call the rectory at 508-679-6732.

All program communications are done via email. Please provide the email(s) to which you would like to receive regular communications.

All new students not previously enrolled in the program must submit a copy of their Baptismal Certificate unless baptized at Holy Name Church. Certificates should be mailed to Holy Name Faith Formation 709 Hanover St. Fall River, MA 02720 or brought to class.

After completing this registration form, please submit your child's registration fee. Payments can be made using cash, check, or Venmo (@HolyName-FaithFormation). Cash and checks should be sent to Holy Name Faith Formation attn. Dorothy Mahoney-Pacheco 709 Hanover St. Fall River, MA 02720. Please make checks payable to Holy Name Parish. When submitting a payment please include your child's first and last name in the memo line.

Form of Payment *

Do you give permission for your child to join the Holy Name Faith Formation Walking Group for the 2023-2024 program year? 

This would allows your child to join any of the following groups:

· A group will meet at the Mary Altar in Holy Name Church immediately following the 8:30 Mass. The group will walk from the church to President Ave. via Madison St., then from President Ave. to Read St. at which point they will enter the school building. At least two adult catechists will accompany this group.

· A group will meet in the Parish Center of Holy Name School at 10:15AM in conjunction with the conclusion and dismissal of Faith Formation classes. The group will walk from Read St. to President Ave., then from President Ave. to Madison St. at which point they will enter the church. All students will be dismissed to parents/guardians for the 10:30 Mass at the Mary Altar. At least two adult catechists will accompany this group.

· A group will meet at the Mary Altar in Holy Name Church immediately following the 10:30 Mass. The group will walk from the church to President Ave. via Madison St., then from President Ave. to Read St. at which point they will enter the school building. Fr. Williams will accompany this group. [This group is only available to students enrolled in the Confirmation program.]

PLEASE NOTE: This is for record keeping purposes. Children of parents/guardians who respond "yes" are not obligated to walk but instead have the option to at any point in time. Children of parents/guardians who respond "no" are NOT allowed to walk at any point in time. To change permissions, please contact Dorothy Mahoney-Pacheco.
