CORE's Service Request Form  
All of CORE's services are confidential. Please fill out this form if you are either a young person (ages 16-29) requesting CORE services (Street Outreach, Harm Reduction Services, or Service Navigation (Case Management) Services ) OR a community member requesting CORE services for a young person. You do not have to be unhoused or income qualify to access our services. 

Some of the resources our Street Outreach (SOAP) and Harm Reduction Distribution (HRD) Services Provides are: 
*Naloxone Distribution/Overdose Ed.
*Resources 
*Safe Sex Supplies
* Harm Reduction/Survival/Hygiene Supplies

Some of the things our Service Navigation Program can help with are                                                                    *Document recovery  *Tribal Enrollment/Navigation  *Public Benefits Enrollment/Navigation   *Housing  *School (Re-enrollment/College)  *Advocacy *Systems navigation *Direct Client assistance as funding allows (rent support, move-in assistance, relocation, document recovery, etc.)


CORE's SOAP Program is for youth and young adults 29 and under. CORE current outreach schedule is Wednesday nights from 5pm-7pm.
