This report is to be completed by the Cooperating Teacher no less than four times during the student teaching experience. Check the number on the rating scale which in your judgement best reflects the student teacher's performance. Please leave a rating blank next to any items which you feel are not applicable. Before SUBMITTING the form, a conference should be arranged between the Cooperating Teacher and Student Teacher to review the report and discuss the student's progress, offering positive reinforcement and suggestions for improvement.

Scale (1-5) ranging from (1) Observable weaknesses in stated area to (5) Demonstrates skills necessary for effective teaching.
