Withdraw from Berkshire Maestros activities
We understand that some students will occasionally need to withdraw from music tuition/ensembles either permanently or temporarily. If this is the case, please complete the following form so that we can cancel your current arrangements. You will need your account number to be able to complete this form. If you cannot find your account number, please contact our admin team to ask for it by e-mailing them using admin@berkshiremaestros.org.uk, or telephoning them using 0118 901 2370.

This form should take less than two minutes to complete and your answers will help us to evaluate the service you have received and improve the provision of music tuition or ensembles. A member of the Berkshire Maestros team will contact you within two weeks to confirm your withdrawal.
