College Curling USA - All Regions - Large Event - Team Roster Reporting Form
This form is for reporting the team rosters for all USA Curling College events that involve six or more teams, such as a Bonspiel. One submission per team per event is all that is required. Event organizers will submit results of play.

This form will submit automatically. If you print off and fill in a hardcopy, please scan and send responses to results@collegecurlingusa.org

Roster names will be checked against the list of registered players. All game results are considered provisional until all players are registered with the United States Curling Association. If a listed player does not register by January 31, 2017, that game will be forfeited, the opposition will be awarded the points earned and the offending team will be awarded 0 points. If both teams have unregistered players listed after the deadline, the game results will be nullified.

RESULTS
The event organizer should act as the "disinterested third party and sign the next section. The team skip should submit this form. Event results will be reported by the event organizer.
Information Verification
By signing below or typing your name, you verify that all of the above information is accurate. If discrepancies are noted in the reporting of the game's results or by the lack of a witness, your match is subject to disqualification. As team skip you must take due diligence to make sure all parties have taken accurate record of the event proceedings before leaving the club where the match has been played.
