The FASD Network of Northern California is currently gathering information on the experience of individuals affected by Fetal Alcohol Spectrum Disorder (FASD) with the Regional Centers across the state of California. Your input could greatly contribute to the understanding of accessibility for Regional Center services for children and adults with FASD in our state.

Anyone who has applied to a Regional Center for their child or adult with FASD, please fill out this form.  We are seeking information on individuals who have been found eligible for Regional Center and under what criteria, as well as individuals who have been denied eligibility and why.  If you have more than one person in your care that has FASD, please fill out additional forms.
