EARLY BIRD SPECIAL** TEAM ENTRY FEE: $500.00 (early-bird deadline: September 15th)
Team Entry Fee after Sept. 15th: $550.00

Entry Fee Includes:
-One (1) four person team participating in the golf tournament
-One (1) Sleeves of ProV1 or ProV1x golf balls per team member
-One (1) door prize ticket per team member
-Lunch provided
-4 closest to the pin contests per course
-1 long drive contest per course
-Range Balls provided prior to round
