The Ugnayan ng Pahinungód UPOU is the official volunteer service arm of the University of the Philippines Open University.

Before volunteering for the activities of the Ugnayan ng Pahinungód UPOU, may we ask you to kindly answer this information sheet first.

This information sheet will ask for your personal information (name, address, birthday, email address, etc.), your volunteer experience, and future volunteer opportunities that you may be interested with. On the last section, we will ask you to answer the DPA form in accordance with the Data Privacy Act of 2012 (RA 10173).
