Dear Parents of Farr West Elementary Choir Students,

Our choir is planning some amazing singing opportunities this year! We plan to perform in a variety of locations.

The school and our PTA would like to share many of these experiences by posting pictures or video recordings of the events that may include students in the choir. These pictures/videos would be posted on the school’s website or made available via a Google link.  These pictures and videos could be publicly online or made available to community members via a Google link. No names or other identifiable information will be shared. 

Please indicate below if you DO NOT give permission for your child to be visible in the choir’s pictures and videos. Permission is assumed if this form is NOT signed below.
