Please complete the following application for consideration into DASH Academy Extended Term '22 -'23. In completing and submitting this form both guardian and applicant agree that all information is current and accurate.

A transcript or report card must be attached to this form or emailed to dashacademyedu@gmail.com in order for the application form to be considered complete.

The first audition dates for the '22-'23 school year will be held on January 19th and February 10th at 1013 E. 15th Street, Plano, Tx. . Once all documents have been received a notification email will be sent with audition details to the email provided below. If the artist applying for DASH Academy Extended Term is unable to attend please email dashacademyinfo@gmail.com for additional audition options.

In consideration of the benefits of instructions provided by The DASH Academy, for my dancer, I, intending to be legally bound, do hereby allow my child to participate in DASH Academy Extended Term audition , and do hereby waive claim and release The DASH Academy, Directors, Faculty, and/or any other instructors, and/or other personnel for claim or liability for any injury, or accident occurring or arising from the instructional program or incidental sponsored activities either on or off these premises. By choosing to allow my child to audition as indicated below and submitting this form, I, the guardian, authorize The DASH Academy to use my dancer’s image in part or whole in publications and/or on the website, Thedashensemble.org and/or any social media associated with The DASH Ensemble. *
