Dear Parents,

Thank you for signing your Son(s) up to be a part of the Divine Mercy Altar Servers. Please speak to your son(s) before filling up this form and ensure that they have expressed a form of interest in joining the Ministry - there is a degree of commitment required in the Servers.

We hope that we can develop a strong partnership to help your son(s) grow in faith while serving God.

There will be trainings prior to your Son(s) Serving on the Altar. There will also be trainings in preparation for Holy Week and Masses of Special Occasion such as Feast Day Masses. We would be grateful if your child could make time for the trainings when scheduled.
